The Housing and Services Resource Center (HSRC), developed by the U.S. Department of Health and Human Services and the Department of Housing and Urban Development, is designed to support individuals with disabilities and their families as they plan for life after high school. It offers a centralized hub of information focused on helping self-advocates explore housing options, access community-based services, and build a future rooted in independence and inclusion.

Description

Through its Focus Areas, the HSRC provides practical tools, training, and guidance to support person-centered planning. Self-advocates can use the site to learn about affordable housing, supportive services, and strategies for transitioning from institutional settings to community living. With real-life examples and easy-to-navigate resources, the HSRC empowers individuals to take an active role in shaping their adult lives.
