About Transition Hub
Transition Hub is here to help you find resources you need
Transition Hub is a group effort. It was put together by multiple parent center organizations throughout the West Coast and Pacific Island communities that work with youth who have disabilities and their families every day.
We are parents, teachers, disability professionals, and self-advocates who are all dedicated to helping youth with special needs live life to their fullest potential.
We know how hard it is for young adults to transition out of high school. Laws, responsibilities, and expectations change along with programs... It can be very confusing.
How Transition Hub came to be
Transition Hub is funded through a grant from the Rehabilitation Services Agency (RSA). The RSA is an arm of the United States Secretary of Education.
The goal of this project is to provide community-specific and relevant resources and information about transition from high school to adult life after high school. This process can seem difficult and overwhelming, but there is good news.
Our communities are full of tools and resources to help youth make this transition! Transition can be a complicated process, but you are not alone. This site provides or shows you how to find tools, resources, and support to help you live your best self.
Everyone involved in this project wants to make sure that those resources get to you!
Available Resources
We have carefully chosen tons of resources from a variety of communities and life experiences. We have also tried to explain them in plain language that everyone can understand. Our goal is to help youth and their families plan for the future with the personal knowledge to make that future a reality.
