What is Transition?
Transition is the time between ages 18-21 when you move from youth to adult services. Starting as young as 16, young people with disabilities begin planning for adulthood. During this time, you'll explore new programs, start making more choices about your life, and build skills for independence. Since this brings many changes - like different rules and responsibilities - early planning helps create a smoother journey to adulthood.

Everyone deserves a
say in their future.
While it is important to understand limitations, it is equally important to value yourself. Everyone has limitations and that is okay! You are a valuable person who has rights. You also have programs available to you. Using those resources is not a bad thing. They are there to help you and you deserve to know how they work.
Know Yourself
Determining what is best for you starts with understanding yourself.

Value Yourself
You matter & you have rights, everyone is different. That's what makes us who we are! Some things might be harder for you, and that's okay.

Plan
Success starts with a plan. Breaking down big goals into smaller steps makes them easier to reach. Take it one step at a time – you've got this!
